At Creekside Family Dental, we believe quality dental care should feel simple, accessible, and personalized. Our Choice Care Membership Plan is designed for patients and families who do not have dental insurance and want an easier way to manage their dental care throughout the year.
With an annual membership, you no longer have to pay full price for many dental expenses. Members receive included preventive care benefits and reduced rates on many services at Creekside Family Dental, from routine cleanings and fillings to cosmetic procedures and crowns.

Membership is effective the 1st of the month in which payment is received.
It is the sole responsibility of the member to schedule and use their appointments within the 12-month membership period.
If appointments are not used, the member will not be entitled to a refund.
Renewal payment is due at the beginning of the same month each year.
The membership fee is guaranteed for the initial 12-month term.
Renewal rates may be adjusted at the time of renewal. However, members will receive advance written notice of any pricing changes before the renewal date.
The Choice Care Membership Plan is not a dental insurance plan.
The plan can only be used at Creekside Family Dental.
The membership is non-transferable.
The plan cannot be combined with any dental insurance coverage.
Discounts under the membership do not apply to any treatment completed before joining.
